Japanese cities generally have a lot of grime, in Tokyo your fingernails will go black if you keep them too long. Also the way they sort out garbage and use a million plastic bags for everything makes a lot of garbage.
Also there's the same old people pissing in the streets you get everywhere else.

Milan is beautiful, at least the city center. But everything outside of that is pretty dirty and - without being racist - I felt pretty threatened by the masses of Africans in the city center. There are 3 of them on every corner trying to sell you selfie sticks or shit like that. And even worse are the ones that don't sell things and just sit around on benches, especially in the night.
Generally a beautiful city, no doubt, but there are some clear downsides.

I just want a big yard where i can plop down a workout shed and shoot bows and guns, 5-10 minute drive to anything i ""need"" on a regular basis, and otherwise just no obscene crime rate. My hometown is actually a good place for all that, and is only a 10-20 minute drive to a 500k city, but my field is niche and has no jobs for entry levels there
