Heard this was awful for the rotator cuff and worse than bent over row for muscle activation.

That's just a different but completely acceptable way of doing Bulgarian split squats you dingus.

I've found it helps knee health a lot

The foot should be further out to place less stress on the knee.

Doesn't hurt your knees any more or less. Try it user, it's a good exercise.
